.The universe was created with slight imperfections...These imperfections are the reason we have structure in the universe today.COSMOLOGY!......
So, It's logical to conclude that the first few black holes are now supermassive black holes and form the center of galaxies.
The galaxies themselves are formed into local clusters clumped around their local biggest galaxy. (More generally their common center of mass)

And the clusters are organized into super-clusters of many 1000's of galaxies which are placed in such a way as to conform to the original density 'blueprint' from the CMBR.

Basically...
The universe was created with slight imperfections...
These imperfections are the reason we have structure in the universe today.

The stars of the early universe were hypergiant stars, too large to form into neutron stars, that went hypernova and thus produced the first super-gravity black holes that helped form the next generation of stars into galaxies. The early stars were well over 100 solar masses, which is beyond the mass that collapses into a neutron star.
