Because Feminism and equal rights for women didn't come into full stride until the last 50 years, or so. Until then it was all men studying these things, so they naturally drew themselves. Things are changing. Most of my textbooks have women in them. Maybe you're using high school text books from years ago, but this is a minor detail, regardless. Women are currently more likely than men to graduate college, so what really should be worked out is getting more equal pay.
On your point about showing a woman in a gym, it may cause issues for some people to show muscles used while bench pressing on a woman.
As for the above question of why men have nipples, it's simply because the genetic code that creates nipples isn't a sex linked chromosome(X or Y). It's not like the first human was a woman, who reproduced asexually, and accidentally created a man.
