The primary structure involves the sequence of amino acids to make the protein (in its polypeptide chain form). it does not affect the structure of the haemoglobin (yet) but affects the sequence.
A change in amino acid affects the shape of the protein, it starts to shape in its secondary structure as alpha coils and beta sheets form. Thus the structure is affected in this stage.
This also of course affects both tertiary and Quatenary structures, but i think the question asks when does it affect the protein STRUCTURE initially, and that is in its Secondary structure.
