Neptune itself is over 2 billion miles from earth, if it just spun out without help, practically nothing, definitely nothing significant. The question is what caused Neptune to spin out? There is obvious something new in our solar system that has potential danger to earth. If it is going sun ward, definite concern

Neptune is part of our solar SYSTEM, a system that is not just made of a bunch of rocks or gaseous balls pursuing their specific orbits. But rather, like any system known, would have individual part functioning and interacting with each other to sustain its quality and its existence, in ways we do not yet understand.
Without Neptune our solar system might not be crippled nor might it be limping. We might not witness a specific or drastic phenomenal disaster like tsunami or earth quakes. But it will be like loosing some one within our social circle, a friend or even a family member.
The earth surface that sustain life is extreme thin and extremely sensitive. It will not tolerate the slightest change in the solar system. Neptune spun out of our solar system or galaxy will be felt.
The solar system would need to readjust its course towards a new equilibrium point which might also change the course of our evolution to an unknown destiny.
What would happen to earth if Neptune spun out of our galaxy will never be known in a specific way. But one thing is sure. It will change our solar system, probably the course of our evolution or even our destiny as human.

Probably nothing. There's a tiny chance that it's outward passage would change the orbit of some comet or asteroid and send it towards Earth, but it's an infinitesimally small chance. Neptune doesn't influence Earth in any way, so its' absence would have no affect on Earth.

Nothing. Neptune is too far away for its gravitational field to affect much of anything.
-
No more Neptune. That's about all.
