Migrane is a paroxysmal disorder characterized by recurrent attacks of cephalgia, with or without associated visual and GI disturbance.

c.migraines are associated with gradual pressure increased in the intestines

I believe its 'a' or 'c'... not sure that why im stuck on this question. The answer to this question will be appreciated.

Actually, it's B. Based on that sentence which uses the term 'attack'. There is no mention of anything related to 'pressure' in the statement and 'disturbance' means anything from pain to diarrhoea to constipation with regard to GI. There is also no mention of the intensity of the headaches/cephalgia and you cannot infer that there is a component of severity from the statement, but unfortunately those poor options mean you have to choose A or B.

The statement in layman terms: Migraine is a 'sometimes there, sometimes not there' disorder characterized by 'repeat episodes of head pain' ....'which may (or may not) have vision symptoms and may make your tummy feel upset or sore'. That is, sometimes there are vision symptoms, sometimes there are tummy symptoms, but neither is required for a diagnosis.

None of the options is great in terms of interpretation of that statement, but B is the least bad interpretation. Don't worry if you get this question wrong by the way... it's a poor and rather pointless question if you ask me... the answers are very unrelated to the essence of the statement.
