I would choose "B" Disturbed Thought Processes

My rationale is that a nursing intervention alone could compensate for A.C,and D, but that disturbed thought processes are indicative of a significant enough mental illness that a physician, preferably a psychiatrist must be included. I believe that most nurses would first obtain medical assistance for B, and formulate a nursing care plan to enhance the patient's coping for the other issues. Best wishes.
