I'll just describe the process that occurs so you can take whatever information you want from it.

Glycogen is a way that the body can store glucose/energy to be used later and lower blood glucose levels.
Basically, your hepatocytes have insulin receptors on them which will be triggered when your blood glucose levels rise then your hepatocytes will try to pull the glucose out of your blood and turn it into glycogen, lipid droplets (fat) or protein

Glucose enters the hepatocytes (in the liver) via GLUT2 transporters. The glucose is converted to glucose-6-phosphate by glucokinase then glucose-1-phosphate is converted to glycogen via glycogen synthase.

Uhh I'm not sure if i understood the last part of that question properly but the transportation of glucose into the hepatocytes sends a negative feedback to stop the action of glycogen phosphorylase which acts to break glycogen back into glucose. This means that the uptake of glucose itself will inhibit the breakdown of glycogen so that blood glucose levels don't get any higher.
