a. only 2 gametes may form instead of four
b. some gamete may have an extra copy of some genes

They might receive an extra chromosome, basically. If nondisjunction occurs during meiosis, it means the chromosomes aren't evenly split (one has too many chromosomes, another doesn't have enough)
