In doing a blood test for narcotics, would a (just) prior transfusion have rid the circulatory system of the drug all together? Or could there still be a trace?

No it will not because Cocaine binds directly to the DAT1 transporter [dopamine transporter]
The dopamine transporter (also dopamine active transporter, DAT, SLC6A3) is a membrane-spanning protein.Regional distribution of DAT has been found in areas of the brain with established dopaminergic circuitry including: nigrostriatal, mesolimbic, and mesocortical pathways.
So a complete blood transfusion will not be effective. For no trace the drug need to be diluted and removed through bodily fluids (mainly urine and sweat) by intake of large quantities of water but if this is done 24-48 hrs before the test it will raise suspicions cause the urine will be overly diluted.
