What are the two functional groups that make up an ester linkage?
what forms when two hydroxyl groups are bonded together?

1. Carboxyl group and Hydroxyl group.
2. Water.
2. Water.

I've just remembered, perhaps what you meant for the second question is glycosidic bond? Maltose is just a product from glucose + glucose, produced when two hydroxyl groups are bonded together. All formation of disaccharide (e.g. sucrose, lactose, maltose) from monosaccharides undergo this process

To your first question. Visually, the best answer would be ketone + ether = ester. However to achieve an ester chemically, you would need to react a carboxylic acid with an alcohol.

OH+OH------H2O+H,it function as the loss of a molecule of water(H2O)
