The most common ion for acid is the hydronium ion H3O+. Many people abbreviate this as H+ however, so you are partially correct! Note however this is only true for acid/base reactions in aqueous (water) solvent. There are a lot of reactions that occur in non-aqueous media as well though.
e.g. H-Br + H2O --> (H3O+) + Br- where the acid in solution is H3O+ because HBr, being a strong acid completely dissociates in water.
A neutralization reaction creates salt and water as products. for example:
e.g. NaOH + HBr -->H2O + NaBr Where NaBr is sodium bromide, a salt!
e.g. H-Br + H2O --> (H3O+) + Br- where the acid in solution is H3O+ because HBr, being a strong acid completely dissociates in water.
