So basically, there is a bit of chance involved but with most infections, pathogens either end up in the blood or in the lymph tissue, where the chances of meeting lymphocytes or macrophages is high. And although there may only be one specific lymphocyte for each infection, there are lots of macrophages and body cells doing antigen presentation so the chance of it coming into contact with one of those is actually pretty high. Also, there are chemicals released by the immune system known as chemotaxants which help draw immune cells to the sites of infection which also help the process along.
