The simplest answer is that nobody knows exactly. Monozygotic (or identical twins) occur when the embryo splits at the beginning of the pregnancy but scientists and doctors have yet to learn why. One theory is that eggs in older women can "break off" rather than complete a normal split and that results in two fetuses instead of one but they've found no evidence to support the idea.
