Question 1
There is going to be a problem for anyone answering this question (including Canadians), because the term "city" is not usually used for census purposes. There are different ways to look at it. Canada ranks its population in terms of "Metropolitan Areas," "Urban Areas," and "Municipalities."
In terms of Metro. Areas and Urban areas, the first, second, and third largest are Toronto, Montreal, and Vancouver, respectively.
In terms of Municipalities, the first, second, and third largest are Toronto, Montreal, and Calgary, respectively.

Question 2
According to another answerer from another question, it is Bytown. But I cannot be sure of this, and I do not know exactly what you are asking tobe honest...were you asking what the original/first name of the city of Ottawa was? Please clarify.

Question 3
Most commonly Lake Superior is considered the biggest freshwater lake in the world in terms of surface area.
According to Wikipedia, though, the largest freshwater lake in the world by measurement of area is Lake Michigan-Huron. These two separately named lakes are actually one continuous, connected body of water. If counted as one lake they are the largest freshwater lake in the world.
But when counted separately, as they usually are, Lake Superior is the largest freshwater lake in the world, and Lake Huron is 3rd largest and Lake Michigan is 4th largest.
In terms of volume, Lake Baikal in Russia is the biggest freshwater lake, as well as the deepest lake (in general) in the world.
